Durable Power of Attorney for Health Care
This is a document which allows your agent to make health care decisions for you, provided you are unable to make those decisions for yourself. The decisions a patient advocate may make on your behalf include, but are not limited to, scheduling medical appointments, facilitating your move into palliative care or hospice (if you are not taking care of yourself), or making the decision to pull the plug.

Clients usually name three to four levels of individuals (i.e., spouse; child, if attained age 18; father; and sibling).
